  2. Enter your email & click submit
  3. Check your email for a confirmation link & click it
  4. It should re-direct you to page when you'll need to:
    1. Provide date of birth
    2. Provide legal name
  5. After entering those 2 pieces of info, find your record & click continue
  6. In the "Desired Behavior" dropdown, select Suppressed.
  7. Click save & you're done!
